A review of the bio-tribology of medical devices SCIE CSCD Scopus

Abstract :

Numerous medical devices have been applied for the treatment or alleviation of various diseases. Tribological issues widely exist in those medical devices and play vital roles in determining their performance and service life. In this review, the bio-tribological issues involved in commonly used medical devices are identified, including artificial joints, fracture fixation devices, skin-related devices, dental restoration devices, cardiovascular devices, and surgical instruments. The current understanding of the bio-tribological behavior and mechanism involved in those devices is summarized. Recent advances in the improvement of tribological properties are examined. Challenges and future developments for the prospective of bio-tribological performance are highlighted.

Abstract :

This study aims to evaluate the reconstructive stability for Paprosky III acetabular defects after total hip arthroplasty using three different reconstruction strategies with trabecular metal (TM) augments. The acetabular bone defects examined were located in the ilium, the sciatic ramus and the pubic ramus. Different scenarios of acetabular reconstructions were simulated, including the non-reconstruction model (NRM), the complete reconstruction model (CRM), the two-point reconstruction model (TRM) and the superior edge reconstruction model (SRM). A primary hip replacement model (HRM) was also investigated to compare the initial stability with different reconstruction models. The gait cycle was incorporated in the model to investigate the dynamic variation within the contact mechanics parameters. By comparing the SRM and the TRM, the acetabular cup translation was more pronounced when the superior defect on the acetabulum remained unfixed. Comparison of the acetabular cup displacement and the interface micromotion of both HRM and CRM demonstrated that the prosthetic implant provided good support for the reconstructed acetabulum. With the use of a press-fit cup, the cup displacement was reduced remarkably, while its Von-Mises stress increased significantly. The results show that the CRM was the best reconstruction option. In terms of acetabular defects, future improvements should focus on the reconstructive stability in stress concentration areas, to ensure no significant stress-shielding or other factors contributing to loosening of the prosthesis.

Abstract :

Purpose: This study aims to investigate the contact behavior of nominal curved surfaces with random roughness. Design/methodology/approach: A deterministic model was applied to investigate the contact behavior. Numerical calculations were conducted on Gaussian and fractal profiles under a range of loading conditions. The deformation behavior is characterized in terms of three regimes including the elastic, elastoplastic and plastic regimes. Findings: A linear relationship was observed between the real contact areas and normal loads, which is mainly governed by the plastic deformation. Surface roughness changes contact behavior by influence the transition of deformation regimes. Rougher surfaces generally demonstrate higher saturated plastic ratios. Originality/value: The contact behavior of nominally curved surfaces with random roughness is understood in terms of the evolution of real contact areas and plastic ratios. Abstract :

The overall objective of this study was to introduce knee joint power as a potential measure to investigate knee joint stability following total knee arthroplasty (TKA). Specific aims were to investigate whether weakened knee joint stabilizers cause abnormal kinematics and how it influences the knee joint kinetic (i.e., power) in response to perturbation. Patient-specific musculoskeletal models were simulated with experimental gait data from six TKA patients (baseline models). Muscle strength and ligament force parameter were reduced by up to 30% to simulate weak knee joint stabilizers (weak models). Two different muscle recruitment criteria were tested to examine whether altered muscle recruitment pattern can mask the influence of weakened stabilizers on the knee joint kinematics and kinetics. Level-walking knee joint kinematics and kinetics were calculated though force-dependent kinematic and inverse dynamic analyses. Bode analysis was then recruited to estimate the knee joint power in response to a simulated perturbation. Weak models resulted in larger anterior-posterior (A-P) displacement and internal-external (I-E) rotation compared to baseline (I-E: 18.4 ± 8.5 vs. 11.6 ± 5.7 (deg), A-P: 9.7 ± 5.6 vs. 5.5 ± 4.1 (mm)). Changes in muscle recruitment criterion however altered the results such that A-P and I-E were not notably different from baseline models. In response to the simulated perturbation, weak models versus baseline models generated a delayed power response with unbounded magnitudes. Perturbed power behavior of the knee remained unaltered regardless of the muscle recruitment criteria. In conclusion, impairment at the knee joint stabilizers may or may not lead to excessive joint motions but it notably affects the knee joint power in response to a perturbation. Whether perturbed knee joint power is associated with the patient-reported outcome requires further investigation.

Abstract :

The elastic loading behaviour of rough surfaces is derived based on the physical understanding of the contact phenomena, where the pressure distribution is analytically obtained without any negative values or convergence problems, thus the evolution of the contact behaviour is obtained in a semi-analytical manner. Numerical results obtained by the proposed approach facilitate the understanding of the contact behaviour in the following aspects: 1) the ratio of contact area to load decreases with an increase in real contact area; 2) normal approach-load relationship is approximated by an exponential decay under relatively small loads and a linear decay under relatively large loads; and 3) average gap shows an exponential relationship with load only in moderate load range.

Purpose: This study aims to develop a multi-material stereolithography (MMSL) technique to directly fabricate a biphasic osteochondral scaffold. Design/methodology/approach: A bespoke prototype MMSL system was developed based on a bottom-up mask projection approach. The system was controlled by a multi-material fabrication algorithm with minimum number of switching cycles during fabrication. A variable-power light source was used to fabricate materials with significantly different curing characteristics. The light-curable poly(ethylene glycol) diacrylate (PEGDA) hydrogel and beta-tricalcium phosphate (β-TCP) ceramic suspension were used for fabricating the biphasic osteochondral scaffold. Findings: The bonding strength of the multi-material interface is shown to be mainly affected by the type of photopolymer, rather than the switching of the materials in MMSL. Lighting power densities of 2.64 and 14.98 mW/cm2 were used for curing the PEGDA hydrogel and the ß-TCP ceramic suspension, respectively. A biphasic osteochondral scaffold with complex interface was successfully fabricated. Originality/value: This study proposes a potential technical method (MMSL) for manufacturing a complex biphasic osteochondral scaffold composing a PEGDA hydrogel/ß-TCP ceramic composite in a time-efficient and precise manner. The designed bone-cartilage scaffold interface and the surface of the cartilage scaffold can be precisely manufactured. © 2018, Emerald Publishing Limited.

零件的多材料组成可改变其机械性能,并增加其他附属功能.通过搭建一种分区旋转槽多材料面曝光增材制造系统,验证了多材料光固化成形工艺,完成了含有两种树脂材料的复杂结构零件的加工.分析了零件的加工质量和加工精度,并进行了多材料界面结合的抗拉和抗剪性能测试.结果表明:所加工的多材料界面在拉应力和剪应力的作用下结合良好.

Abstract :

Ultra-low-wear polyethylene (ULWPE) is a new type polyethylene made by experts who are from China petrochemical research institute, which is easy to process and implant. Preliminary test showed it was more resistant to wear than that of Ultra-high-molecular weight polyethylene (UHMWPE). The purpose of the research is to study biocompatibility, bio-tribological properties and debris characterization of ULWPE. Cytotoxicity test, hemolysis test, acute/chronic toxicity and muscular implantation test were conducted according to national standard GB/T-16886/ISO-10993 for evaluation requirements of medical surgical implants. We obtained that this novel material had good biocompatibility and biological safety. The wear performance of ULWPE and UHMWPE was evaluated in a pin-on-disc (POD) wear tester within two million cycles and a knee wear simulator within six million cycles. We found that the ULWPE was higher abrasion resistance than the UHMWPE, the wear rate of ULWPE by POD test and knee wear simulator was 0.4 mg/10(6)cycles and (16.9 +/- 1.8)mg/10(6)cycles respectively, while that of UHMWPE was 1.8 mg/10(6)cycles and (24.6 +/- 2.4)mg/10(6)cycles. The morphology of wear debris is also an important factor to evaluate artificial joint materials, this study showed that the ULWPE wear debris gotten from the simulator had various different shapes, including spherical, block, tear, etc. The morphology of worn surface and wear debris analysis showed that wear mechanisms of ULWPE were adhesion wear, abrasive wear and fatigue wear and other wear forms, which were consistent with that of UHMWPE. Thus we conclude that ULWPE is expected to be a lifetime implantation of artificial joint.

Abstract :

Laser metal direct forming technology was applied to manufacture customized titanium implants, and then titanium implants can be directly formed according to their CAD models. This method possesses a short manufacturing cycle. Additionally, the customized titanium implants manufactured in this method have good mechanical properties. The influences of curvature effect and inclination angle on the geometric precision of laser metal direct forming parts were studied for the establishment of the model design criteria. Therefore, the formed titanium implant could achieve high precision. Both mechanical properties and chemical composition of the manufactured customized titanium implants comply with relevant national and pharmaceutical industry standards. Two cases of customized titanium implants manufactured by laser metal direct forming have been successfully applied in clinical and have achieved good results.
